McKinney-Vento (Definition, Identification, Services etc.)

Disaster Response

TEA Sample Student Residency Questionnaire (SRQ)

The SRQ has been updated (10/2026) to provide a more comprehensive and family-friendly process for identifying students who may qualify for McKinney-Vento protections and services. The revised template includes enhanced housing-related questions, clearer living situation options, and a new district-use documentation page designed to support eligibility determinations and required follow-up actions.

Why it matters:

  • Supports early identification of students experiencing homelessness.
  • Promotes consistent McKinney-Vento eligibility determinations across campuses and districts.
  • Helps districts document required actions and services following eligibility determinations.
  • Aligns identification and follow-up processes in a single resource.

🚨 Important: Page 2 contains a new District Use Only form that includes eligibility determination documentation and a post-identification checklist of required next steps and supports. This page is intended for district records and implementation purposes. The District Form should NOT be printed on the back of the parent-facing SRQ. Instead, districts should print and maintain it as a separate document for staff use. The parent or caregiver should complete only the SRQ portion, while district staff use Page 2 to document eligibility decisions, service coordination, transportation considerations, educational stability discussions, and other required McKinney-Vento follow-up actions.

How to use it:

  • Provide Page 1 (SRQ) to the parent, guardian, or unaccompanied youth.
  • Review responses to determine whether additional clarification is needed.
  • If needed, utilize the TEHCY Follow-Up Intake Form to gather additional information and support the eligibility determination process.
  • Once eligibility is determined, use the separate District Form (Page 2) to document the determination and track required services, supports, referrals, and compliance activities.
  • Maintain the District Form with district records as supporting documentation.
